The following information is required by Accenture:
- Name
- Work Location
- Residence
- Mobile Number
- Time of shift
- Assessment Response
- Symptoms
- Exposure to a COVID-patient
- History of Travel
- Negative RT-PCR Test Result
- Said Personal Information and Sensitive Personal Information (hereafter referred to as “PI”, will be recorded, reviewed, transferred to and between (and to other countries), processed or otherwise used by, Accenture for the following purposes:
- Effective control of the COVID-19 infection
- Comply with RA 11469, Bayanihan to Heal as One Act, related regulations
- Return-To-Office (RTO) Health Check
- For data analytics,
- for contact tracing and
- management of asymptomatic and symptomatic employees in the workplace.
- The PI will be processed in accordance with the Data Privacy Act of the Philippines, Accenture’s Global Data Privacy Policy 0090 and Global Data Privacy Statement1 and existing laws and regulations specifically the DTI, DOLE and DOH-issued guidelines. All personal information collected pursuant to this document shall be retained for 30 days, unless Accenture has another legitimate ground to retain the same.
- Accenture is a global organization and may transfer your information to other countries in which Accenture’s worldwide organization does business and to vendors globally. All such vendors are subject to contractual obligations in relation to the processing of your personal information. Accenture also has internal policies in place to ensure an equivalent level of protection is in place across the organization. Access to your personal information will be limited to those Accenture employees or vendors who have a need to know the information for the purposes described in this document and its Global Data Privacy Statement. These include, but are not limited to, ActiveOne Health Inc. Accenture may also transfer your information to DOH and its partner agencies, as well as LGUs to conduct effective contact tracing and assist the government in providing effective response during COVID-19 pandemic.
- Under the Philippines’ Data Privacy Act of 2012, you have rights as a data subject. These include the right to access, correction and object to the processing of your personal information, to be indemnified for violations of my rights under the Act, as well as the right to lodge a complaint before the National Privacy Commission.
Further, Accenture’s Global Data Privacy Policy 0090 and Global Data Privacy Statement explains your rights to inquire about access and rectify or erasure of your data or restriction of processing and object to its processing as well as the right to data portability in accordance with applicable data privacy laws.
