Human services convergence is both a cause and an effect. As a cause, it is an unprecedented joining of multiple forces of change that are reinventing the human services landscape. As an effect, it refers to coordinated, collaborative and cost-effective health and social care models that deliver better outcomes to people. For convergence to scale and become sustainable, it must be driven “from the top” through legislation and leadership and “from the bottom” through firsthand experiences and frontline action.
