Interoperability Showcase DemonstrationPatient Centered Collaborative CareMarch 4-6, 2013La Nouvelle Ballroom – Level 2, Ernest N. Morial Convention Center
This recent solution extends Accenture’s Connected Health Platform by providing clinicians with access to critical information so that clinicians can effectively manage transitions from one care setting to another. The technology solution provides multiple channels to engage patients and their families with their care teams to support the needs of patients with chronic diseases, episodic acute events or coordinating wellness or illness management over time.
Interoperability Showcase Theatre DemonstrationThe Art of KISSing: Standards Based OperabilityMarch 4, 1:30pm - 2:00pmLa Nouvelle Ballroom – Level 2, Theatre B, Ernest N. Morial Convention Center
A successful health information exchange starts with standards-based interoperability at its core. Exchanging bits and bytes—internally or externally—requires that everyone involved has a profound and quantifiable understanding of the syntax, semantics, context as well as the privacy of the data and its subjects. Explore how the development of healthcare information technology standards have become increasingly complex and making it all but counter-productive to enabling standards-based information exchange and undermining our shared goal of positive patient outcomes. Reducing this complexity requires a major reevaluation of the standards community to separate out truly complex tasks from those that are merely difficult.

Accenture will demonstrate:
Accenture Clinical Data CreatorThis new web-based application will allow health IT projects to create and test large quantities of hypothetical clinician information and standardize for the consumption of other healthcare technologies (e.g., BI, analytics). The solution will allow users to create various clinical scenarios from demographic parameters, clinical terminology and message structures. The solution also contains various templates that will introduce the rate and types of errors, such as semantic and syntactic, for the data sets.
Achieving the Clinical Services VisionWith a collaborative approach unique to each stage of the EMR journey, organizations can realize the full value of an EMR investment from implementation to clinical optimization. An experienced team with proven, flexible, scalable solutions results in reducing delivery risk, meeting time and cost targets, and establishing a framework to achieve results. With such a powerful tool, data driven insights can now can help lower costs and help improve outcomes.
Connected Health Diagnostic Tool: Progressing on the Journey towards Connected HealthThis tool assesses the readiness and progress of using healthcare IT within an integrated health delivery system. Accenture’s new diagnostic analyzes the capabilities and adoption of six dynamics of connected health to assess the potential value and maturity of healthcare IT systems and investments.
Continuity of Care for Proof of ConceptThis recent solution extends Accenture’s Connected Health Platform by providing clinicians with access to health information to help clinicians effectively manage transitions from one care setting to another. The technology solution provides multiple channels to engage patients and their families with their care teams to support coordinating wellness management over time.
Patient Navigator Mobile ApplicationThe Patient Navigator mobile app addresses the needs of lay health workers who help patients, in particular members of under-served communities, make better use of health services. Patient Navigators identify barriers to healthcare such as financial, logistical, and cultural barriers, and work with the patient to overcome them. The mobile app provides navigators more flexibility so they can: 1) define plans to resolve barriers specific individuals face ; 2) identify and connect to community resources to assist in eliminating barriers, and 3) collect data transparently that can be used to measure the value of patient navigation.
