This recent solution extends Accenture’s Connected Health Platform by providing clinicians with access to critical information so that clinicians can effectively manage transitions from one care setting to another. The technology solution provides multiple channels to engage patients and their families with their care teams to support the needs of patients with chronic diseases, episodic acute events or coordinating wellness or illness management over time.

A successful health information exchange starts with standards-based interoperability at its core. Exchanging bits and bytes—internally or externally—requires that everyone involved has a profound and quantifiable understanding of the syntax, semantics, context as well as the privacy of the data and its subjects. Explore how the development of healthcare information technology standards have become increasingly complex and making it all but counter-productive to enabling standards-based information exchange and undermining our shared goal of positive patient outcomes. Reducing this complexity requires a major reevaluation of the standards community to separate out truly complex tasks from those that are merely difficult.
