The panel will discuss how care coordination minimizes adverse effects, mitigates the side effects of treatments and minimizes the negative impacts to informal care givers.Each panelist represents a different point of view in the healthcare continuum and will demonstrate how best practices, health IT tools, care protocols and training are improving compliance and outcomes for cancer patients. They will also discuss how eHealth tools can be used to relay information to providers regarding a patient's pain. Panelists will review self-management tools, highlight the barriers to national adoption and provide examples of how to convince people to adopt these tools.
Accenture and the Harold P. Freeman Patient Navigation Institute are teamed up to equip more people with the skills to get jobs as patient navigators, who provide one-on-one guidance and assistance to patients dealing with the healthcare system. The national effort, is part of Accenture’s corporate citizenship initiative Skills to Succeed.
